These are Documents you need to
Download (they are in word AND PDF format), fill and send back to the school. Please write to
Elisabetta Mendler at: admissions@uwcad.it with any questions.
PDF Format - if you have problems viewing this format, proceed as follows:
* right click on the "Download" link
* choose "Save Object as.../Save target as..."
* specify the correct path on your disk to the folder where you wish to save the
file.
N.B. All items below written in orange must be completed and
returned to the college
before arrival. The medical form and the Privacy document
only need to be done in either English
or Italian depending on which you prefer. Do not
include actual Xrays with the medical form, the written results of the exam are sufficient.
